Introducere
n aceast lucrare se face o analiz teoretic a diverselor mecanisme de finanare a sistemelor de sntate din Europa, pentru evaluarea accesului la serviciile de sntate. Sntatea cetenilor este o prioritate fundamental .Politica n Europa n domeniul sntii completeaz politicile naionale pentru a se asigura c toate persoanele care locuiesc n rile din Europa au acces la asisten medical de calitate. n toate rile din Europa, guvernele sunt implicate n finanarea ngrijirii medicale, cele mai multe state folosesc un sistem combinat ntre contribuiile la asigurrile sociale i finanarea guvernamental direct a sntii. Rolul jucat de asigurrile private difer mult de la o ar la alta, ns, n general, acestea reprezint mai degrab un supliment dect un substituent al sistemului principal de sntate.. Trebuie menionat faptul c, n Europa, o pia a asigurrilor de sntate voluntare care s acopere co-plile nu este foarte dezvoltat, (cu excepia Franei), dar o parte nsemnat a populaiei beneficiaz de ele pentru costurile ngrijirii n ambulatoriu, n Austria, Belgia, Frana, Irlanda, Spania i Portugalia. Acest tip de asigurri poate s includ o mare parte din costurile pentru ngrijirea primar, medicamente, analize, specialiti, transport, maternitate.Organizarea i administrarea ngrijirilor de sntate intr n responsabilitatea fiecrui stat n parte. Politica rilor din Europa n domeniul sntii determin economii importante prin partajarea resurselor i sprijin s se gseasc soluii pentru provocri comune precum: ameninrile la adresa sntii, factorii de risc asociai cu bolile cronice sau impactul creterii speranei de via asupra sistemelor de sntate. Accesul la serviciile de sntate n Europa
1.1 Literatur de specialitate O stare bun de sntate este un element esenial al bunstrii umane (Alber iKohler, 2004), reprezentnd o valoare n sine. La nivel individual, o stare bun de sntate reprezint o component important a capitalului uman, permind oamenilor s i desfoare activitile, s i ndeplineasc elurile, s aib o via complet i s fie membri activi ai societii (Mrginean i alii, 2006). La nivel social, o stare de sntate ridicat este un element-cheie al capitalului uman al fiecrei ri, contribuind la competitivitatea ei fa de alte ri (Alber i Kohler, 2004). ntr-o economie, nivelul cheltuielilor cu sntatea, ca i al celor cu educaia, reprezint un indicator fidel al nivelului de dezvoltare. rile cu economii avansate aloc resurse importante pentru finanarea sectorului de sntate, ca premiz pentru o dezvoltare sustenabil pe termen lung. Sntatea este rezultatul unei combinaii complexe de factori cu relevan la nivel individual i la nivel macro (Precupeu, 2008: 141). La nivel individual, starea de sntate depinde de numeroi factori interrelaionai, precum: motenirea genetic, poziia social, opiunile n ceea ce privete stilul de via (ibidem), comportamentele, atitudinile i valorile adoptate cu privire la starea de sntate. Phillips (2006) consider c o societate se poate departaja de o alt societate n ceea ce privete calitatea vieii, dac oamenii din aceea societate triesc mai mult, mai sntos i au vieii mai mplinite dect ntr-o alt societate. Autorul prezint teza lui Richard Wilkinson n ceea ce privete diferenele privind starea de sntate n diferite societi, mai ales n societile n curs de dezvoltare. Wilkinson (1996) subliniaz n lucrarea sa Unhealthy Societies c starea de sntate a unei populaii dintr-o societate nu este determinat doar de bunstarea economic, ci i de inegalitatea social a veniturilor i de coeziunea social (apud Phillips, 2006). Ipoteza de baz a lui Wilkinson este c un nivel mai ridicat al inegalitii veniturilor determin o inegalitate mai ridicat n ceea ce privete sntatea (idem).Chiar dac, n modelul lui Lalonde (Minc D., Marcu M., 2004) serviciile medicale intervenind cu o pondere doar de 15-20%, n influena sntii, nu trebuie neglijate. Romnia a avut, pn n 1997, un sistem medical puternic centralizat, alimentat financiar doar de ctre bugetul de stat i coordonat de ctre Ministerul Sntii. Oficial, serviciile erau oferite n mod gratuit, ns subfinanarea sistemului o lung perioad de timp a dus la scderea calitii serviciilor i transferul unei pri a costului lor ctre populaie.1.2 Metodologia cercetrii n ceea ce privete metodologia cercetrii, metoda folosit este reprezentarea grafic a indicatorilor, analiza cluster i regresia. Cheltuielile pentru sntate includ: asigurarea de servicii medicale (de prevenie i curative), planning familial, nutriie, ajutor medical de urgen.
Fig.1 Surs: http://databank.worldbank.org/data/views/ Datele indic o cretere a nivelului cheltuielilor pentru sntate, n anul 2013, n comparaie cu anul 2007. n 2007, UE a adoptat Strategia n domeniul sntii, astfel a asigurat finanarea unui numr de peste 750 de proiecte individuale i granturi de funcionare, sprijinind o gam larg de iniiative. Alte surse de finanare UE, precum fondurile structurale i programele-cadru de cercetare, contribuie, de asemenea, la ndeplinirea prioritilor din domeniul sntii. n Europa, ponderea cheltuielilor cu sntatea n produsul intern brut (PIB) a crescut continu n perioada 2007-2013. Se observ o cretere semnificativ pentru trile dezvoltate: Suedia, Olanda, Belgia, Irlanda, Finlanda. Datele la nivel internaional indic faptul c procentul din PIB dedicat sntii n Romnia, este mult mai mic dect cele din majoritatea rilor europene, creterea cheltuielilor totale cu sntatea fiind una marginal, fiind situat pe ultimul loc la acest capitol.
n Romnia cheltuielile cu sntatea prezint mici fluctuaii, avnd o uoar evoluie.

Din Scatter Plot se oberv c intre cele dou variabile, exist o legtur liniar, direct. Intensitatea legturii dintre cele doua variabile a fost determinate prin calculul coeficientului de corelaie, prin extragerea radicalului de ordin 2 din indicatorul R-squared = 0.997, conform output-ului din SPSS. Testarea semnificaiei coeficientului de corelaie liniar s-a realizat utiliznd testul t. Estimarea parametrilor modelului de regresie simpl: pe baza calculelor estimatorilor parametrilor au fost calculate valorile estimate ale variabilei y, y = 26.853 +0.58X . Coeficentul de regresie b= +0.58, are o valoare pozitiv, rezult o legtura direct inrte cele doua variabile. Modelului de regresie este valid, deoarece valoare pragului de semnificaie sig = 0.00 < 0.05, ndeplinind ipoteza H1 cand modelul este valid.ANOVAa

Un alt indicator care prezint accesul la serviciile de sntate Romnia, este ponderea naterilor asistate de personal calificat, acesta este un indicator care msoar accesul persoanelor la serviciile de sntate i se obine prin raportul procentual dintre numrul copiilor nscui cu asistent medical specializat din cadrul sistemului sanitar i numrul total al copiilor nscui intr-un an.
Fig.3 Surs: http://databank.worldbank.org/data/views/Din Figura 3, se observ c indicatorul ponderea naterilor asistate, prezint un trend descrescator.Anul 2000 prezint scderea cea mai semnificativ, deoarece trecerea la sistemul de asigurri a lsat o parte din populaie, fr servicii. Din pcate cea mai mare parte a pupulaiei rurale din Romnia, are un acces dificil la serviciile publice de sntate, multe dintre sate neavnd nici mcar uniti de asisten primar( medic de familie). O parte a populaiei din marile orae beneficiaz de servicii spitaliceti de o calitate ridicat, n contextul naional, pe cnd cea mai mare populaie rural nu beneficeaz dect de asistent primar, n condiii minime sau subminime de dotare a dispensarelor. n conditiile unei populaii srace sau izolate din punctul de vedere al drumurilor de acces, transportul celor din rural pn la servicii spitaliceti reprezint o problem pentru acetia, prin costul sau posibilitile de deplasare necesare.
Fig.4 Surs: http://databank.worldbank.org/data/views/ Din Figura4, se oberv c indicatorul ponderea naterilor asistate de personal calificat, previzionat pe o perioad de 20 de ani, prezint o evoluie descendent, prin urmare accesul la serviciile de sntate pentru toata populaia, nu o sa mai fie un drept social la care are acces toi cetenii, o sa fie un bun individual, la care trebuie sa plteti costurile specifice serviciilor dorite. n prezent, n Europa cheltuielile n sectorul public, au o pondere semnificativ mai mare dect cele din sistemul privat. n Europa Cheltuielile din sistemul public de sntate (% din PIB) includ cheltuieli guvernamentale (centrale i locale), bugete, mprumuturi externe i granturi (inclusiv donaii din partea ageniilor internaionale i organizaiilor non-guvernamentale) i fondurile asigurrilor obligatori de sntate.
Fig.5 Surs: http://databank.worldbank.org/data/views/ n rile Europene, se observ creterea semnificativ intre anii 2007-2011 pentru trile din Nordul-Europei (Danemarca, Norvegia, Suedia, Finlanda, Olanda). Raportate la produsul intern brut, cheltuielile publice cu sntatea ale Romniei sunt ntr-adevr reduse in perioada 2011-2013, nregistrnd niveluri asemntoare cu celelalte state central i est-europene (Ungaria, Slovacia, Lituania, Estonia, Polonia, Republica Ceh). n majoritatea rilor din Europa, principala surs de finanare a cheltuielilor publice cu sntatea sunt asigurrile sociale de sntate. Din punct de vedere al cotelor de contribuie la asigurri sociale de sntate, Romnia -cu 10,7%-se situeaz printre rile cu cele mai reduse niveluri de impozitare din Europa. Pentru comparaie, n Cehia contribuiile ajung la 13,5% din salariul brut, n Slovacia la 14%, n Slovenia la 12,9%, iar n Bulgaria la 16%. n Romania, principalele venituri publice destinate sntii sunt contribuiile de asigurri pltite de angajatori i angajai/ pensionari/ persoane fizice autorizate. n anul 2011 acestea au valorizat 2,6% din PIB. Pe lng asigurrile sociale de sntate, sistemul public de sntate beneficiaz i de venituri din accizele pe tutun i buturi alcoolice (denumite informal taxa pe viciu) i din impozitul pe veniturile realizate de productorii, importatorii i deintorii autorizaiilor de comercializare din vnzarea medicamentelor a cror contravaloare este suportat, integral sau parial,de FNUASS (cunoscut ca taxa clawback). n anul 2011, taxa pe viciu a generat venituri de 1,2 miliarde de lei, iar taxa clawback de 0,24 miliarde lei, ambele la bugetul Ministerului Sntii, dar problema veniturile n scdere, ca urmare a diminurii cotei de contribuie la 10,7% i crizei economice, nu a fost rezolvat.
Pe lng aspectele financiare ale sistemului de sntate, considerm de o importan major i redefinirea relaiilor dintre finanatori, furnizorii de servicii i pacieni. Deoarece actualul cadru legislativ i instituional -n care principalii actori aparin sectorului public -nu mai poate face progrese majore n privina calitii i ateniei acordate pacienilor, analizm propunerile de reform prin care sectorul privat s dobndeasc un rol semnificativ n sistemul de sntate din Romnia.
Cheltuielile private (% din PIB) includ cheltuielile directe fcute de familii (din buzunar), asigurri private, fondurile oferite de instituiile nonprofit pentru pacieni i plile directe fcute de corporaiile private.
Fig.6 Surs: http://databank.worldbank.org/data/views/ Asigurrile private nu joac un rol important n rile Europene, cum se intmpl n Elveia. Din date se observ o uoar cretere a cheltuielilor n perioada 2007-2013. De regul, n rile UE asigurrile voluntare de sntate ofer o acoperire suplimentar pentru servicii neacoperite parial sau integral de cele sociale. Pe de alt parte, acestea pot s ofere i un acces mai rapid la servicii i o ofert mai diversificat. Distincia ntre asigurrile complementare i cele suplimentare nu este foarte clar. n Germania, Olanda sau Spania sunt grupuri din populaie care ori nu sunt acoperite de sistemul social de asigurri de sntate, ori pot opta s ias din acesta i s aleag o asigurare privat. n Germania, asigurrile sociale de sntate sunt obligatorii pentru persoanele care au un venit mai mic de 3 850 pe lun sau 40 500 pe an. Pentru cei cu venituri mai mari dect sumele menionate, asigurrile sociale nu sunt obligatorii, fiind vorba de 9% din populaia rii n 2007. n Germania exist 52 de case de asigurri private care ofer o alternativ la sistemul social sau posibilitatea unor asigurri complementare celor inclui i n sistemul social. Datorit competiiei directe cu asigurrile din sectorul public, n Germania, sectorul privat ofer tipuri diversificate de asigurri substitutive, care pot duce la un acces mai rapid i posibilitatea de a alege din mai multe servicii. Asigurrile complementare ofer acoperire pentru servicii neincluse n sistemul statutar. Acest tip de asigurri pot s includ o parte important din costurile pentru ngrijire primar, medicamente, teste, specialiti, transport, perioada de maternitate. Nivelul rambursrilor variaz de la o ar la alta, precum i n funcie de pachetul de asigurare ales (Elias Mossialos et al., 2002)
n Frana, dei 95% din populaie este inclus n sistemul social de asigurri, 90% contribuie la asigurri suplimentare. De asemenea, conform unor legi din 2006, persoanele cu venituri mici sunt incluse fr plat n sistemul de asigurri suplimentare. Politicile de asigurare centrate pe grupuri sunt preferate politicilor individuale, datorit costurilor implicate.
Fig.7 Surs: http://databank.worldbank.org/data/views/Din grafic se observ diferena semnificativ dintre sectorul privat i sectorul public n anul 2013. Cauzele nedezvoltrii asigurrilor private voluntare sunt multiple: pachetul de servicii de baz este prea amplu, reglementrile sunt sumare, cultura asigurrilor private voluntare este firav, societile de asigurri nu au dezvoltat oferte atrgtoare, nu exist stimulente fiscale nici pentru populaie, nici pentru asigurtori. Asigurrile voluntare ar trebui contractate numai pentru servicii din afara pachetului de baz, pentru a evita migrarea contribuabililor nstrii din sistemul de asigurri sociale.
2. Principalele tipologii ale sistemelor de sntate n Europa

modelul serviciului naional de sntate - tip Beveridge;
modelul sistemului de asigurri sociale de sntate - tip Bismark;
modelul sistemului centralizat de stat - tip Semasko.
Sistemul naional de sntate. Acest model este cunoscut i sub denumirea de Beveridge model i este folosit n Marea Britanie, Italia, Grecia, Finlanda, Spania, Norvegia, Suedia. Principalele caracteristici legate de aspectul financiar al acestui model sunt: Populaia are acces liber la serviciile de sntate bazate pe taxele obligatorii din venituri, deci exist un grad ridicat de echitate social. Resursele financiare provin din colectarea de taxe pe venituri, iar sistemul este gestionat de ctre stat. Cei care ofer servicii medicale n ambulatoriu sunt angajai ai statului. Plile ctre prestatori sunt fcute prin salarii i n funcie de numrul pacienilor. Prestatorii de servicii medicale secundare au la dispoziie un buget general. Sistemul de asigurri de sntate (Bismark model) este un sistem inspirat de legislaia german. Este folosit n multe ri membre ale UE, cum ar fi Frana, Germania, Austria, Belgia, Olanda i Romnia. Caracteristicile acestui model sunt: Resursele financiare sunt, n principal, reprezentate de contribuiile obligatorii pltite de angajai i angajatori. De asemenea, exist resurse care provin din subvenii de la bugetul de stat (local sau naional) sau alte tipuri de subvenii. Instituiile care administreaz fondurile de asigurri sunt nonprofit. Gestionarea i folosirea fondurilor de asigurri sunt fcute la nivel naional i prin direcii locale. n Romnia, fluxul plilor are urmtoarele componente: 1. Contribuii ale angajailor i angajatorilor la fondul de asigurri de sntate. 2. Plai ctre cei care ofer servicii medicale fcute de Casele Judeene de Asigurri de Sntate. 3. Contribuiile Caselor Judeene de Asigurri Sntate la fondul de redistribuire. 4. Transferuri de la Casa Naional de Asigurri de Sntate ctre Casele Judeene. 5. Transferuri de la bugetul de stat ctre prestatorii de servicii medicale, pentru susinerea programelor naionale de sntate. 6. Pli directe pentru servicii i medicamente.Alocarea din PIB pentru sntate n Europa. Sistemul centralizat de asigurri de sntate de tip Semasko, are urmtoarele caracteristici: 1.Sursa de finanare: impozite, taxe generalecare formeaz bugetul de stat; 2. Controlarea procesului de vnzare cumprare se face n plan teritorial prin programare centralizat i execuie stadial;3. Accesul la serviciile medicale pare gratuit,dar este eronat (nu este pltit de pacient). 4. personalul medical nu obine prin munc date suplimentare;5. competiia este absent deci este neperformant; 6.calitatea actului terapeutic este afectat de insuficiena financiar.Sistemul Semasko mai funcioneaz n ri care au avut un sistem economic centralizat n Europa Central i de Est.
2.1 Analiza Cluster
Pentru a fotografia accesul la serviciile de sntate n Europa, se grupeaz rile dup urmtoarele variabile: numele rilor, cheltuieli cu sntatea/locuitor, PIB/locuitor, sperana de via la natere i natalitate. Pentru prelucrarea datelor s-a folosit programul SPSS 20. Anul de referin pentru analiza este 2013, pentru 31 de ri din Europa. Metoda folosit pentru analiz, este analiza cluster ( ierarhic i neierarhic). Scopul analizei cluster este reprezentat de clasificarea datelor (a observaiilor sau a formelor) n structuri informaionale semnificative, relevante, cunoscute sub denumirea de clase, grupe sau clustere. O noiune esenial utilizat n analiza cluster este, prin urmare, cluster-ul. Un cluster se definete ca o submulime a mulimii iniiale de obiecte (observaii) care au proprietatea c gradul de disimilaritate dintre oricare dou obiecte ce aparin cluster-ului este mai mic dect gradul de disimilaritate dintre oricare obiect aparinnd cluster-ului i orice obiect ce nu aparine cluster-ului respectiv. Este necesar a fi menionate o serie de precizri de ordin tehnic. n primul rnd pentru clasificarea ierhic se folosete metoda Ward. Aceast metod este considerat a fi cea mai eficient i mai performant dintre toi algoritmii de clasificare ierarhic deoarece este singura care trateaz explicit problema omogenizrii claselor, adic a minimizrii variabilitii intracluster: la fiecare pas, se comaseaz cele dou clustere pentru care variabilitatea cluster-ului rezultat este cea mai mic din toate posibilitile de comasare . n al doilea rnd, pentru evaluarea distanei (disimilaritii) dintre obiecte (rile din Europa) sau dintre clustere va fi utilizat distana Manhattan. Distana Manhattan, numit i distan rectangular, distan City-Block sau norm de tip L1, se calculeaz ca sum a valorilor absolute ale diferenelor coordonatelor celor dou obiecte sau celor dou variabile analizate Argumentul funciei de optim este chiar distana Ward. Aplicarea acestei tehnici asupra setului nostru de date a generat (figura 2) rezultatele prezentate n dendograma clasificrii pentru cele 31 ri analizate (utiliznd metoda de agregare a lui Ward). Din Dendograma generat n Figura 2, se sesizeaz n linii mari existena a 2 clustere. Avem pe de-o parte un cluster format din Belgia, Finlanda, Danemarca, Suedia, Frana, Marea Britanie, Olanda, Norvegia, Austria, Elveia, Italia, Spania, Portugalia i Germania i un altul format din Polonia, Slovenia, Croaia, Romnia, Bulgaria, Cehia, Ungaria, Letonia, Estonia, Lituania i Ucraina.
Se observ c Elveia este difereniat din primul cluster, un impact major este datorat serviciior de sntate performante, avnd cel mai bun sistem privat din Europa. n Elveia fiecare rezident trebuie s se asigure n sistemul de asigurri de sntate n decurs de trei luni de a natere sau de la sosirea n Elveia. Asigurarea este individual: persoanele trebuie s contacteze fondul de asigurri pe care si l-au ales. Alocaia zilnic de asigurri de sntate este facultativ, doar dac un contract individual de munc sau un accord colectiv de munc prevd asigurarea obligatorie. Dup cum se poate observa, la nivelul fiecrui cluster exist un nivel comparabil de dezvoltare, economic i social. n cadrul dendogramei, rile din Clustererul 1, la fel cum se intmpl i n realitate sunt mai dezvoltate din punct de vedere economic dect cele din Clusterul 2. Indivizii din primul cluster se deosebesc foarte mult de cei din clusterul 2, acest lucru se poate urmri i din distana dintre cele dou grupe.

Cu ct aceast distan este mai mic, cu att rile sunt mai apropiate din punct de vedere al similitudinilor i au anse mai mari s formeze un cluster, cu ct distana euclidian este mai mare, cu att rile sunt mai ndeprtate, mai diferite. Indivizii din primul cluster se deosebesc foarte mult de cei din clusterul 2, acest lucru se poate urmri i din distana dintre cele dou grupe.

Tabelul ANOVA arat ce variabil contribuie mai mult la obinerea soluiei. Variabila cu valoarea coeficientului F mai mare face diferena ntre clustere, aceasta fiind variabila accesul la serviciile de sntate.
n tabelul Final cluster centers se prezint centrele finale ale clusterelor care sunt calculate ca medii pentru fiecare variabil din cluster. Se oberv o superioritate numeric semnificativ n clusterul 1 , pentru variabilele cheltuieli cu sntatea i gpp/locuitor, fata de clusterul 2.

n urma acestei analize, accesul la serviciile de sntate este repezentat de variabila: cheltuieli de sntatea pe cap de locuitor. rile din Europa de Vest au un sistem de sntate mult mai performant dect trile din Est-ul Europei. Un factor reprezentativ pentru aceast rezultat, este datorat si fostului regim politic, astfel rile din spatele cortinei de fier prezint o situaie mult mai slaba din toate punctele de vedere att economic, ct i social. n urmtoarea analiz, se prezint evolut cheltuielilor de sntate pe cap de locuitor .

Principala concluzie care se desprinde este aceea c fostele ri socialiste sunt situate sub media european, n timp cerile dezvoltate ale Europei cheltuiesc cu sntatea peste media european. Alocrile de mic nsemntate acordate sntii sunt din nou evideniate n cazul rilor cu economie n tranziie de la cea centralizatctre economia de pia, care n perioada comunismului au folosit drept model al sistemului propriu de ngrijiri de sntate, modelulsanitar sovietic Semasko. Cu anumite particulariti de la ar la ar, toate statele foste socialiste se situeaz inferior rilor industrializate europene. Acest fapt se traduce printr-o stare de sntate a populaiei nesatisfctoare, cu ample urmri la nivelul productivitii muncii i al dezvoltrii economice.
