CASTIG AD FRACTURE BRACIG BY Augusto Sarmiento, M.D. & Loren Latta, PhD Dr. Sarmiento & Latta’s new 100 page fracture casting and bracing manual and three companion DVD’s will take you through the steps of conser- vative fracture treatment and the application of casts and braces. Included with the manual is a 100 question exam approved for continuing education hours. Figure 2.6. If the pa- tient leans on the elbow it can cause a varus angulation to occur. Whenever possible, the cast should be removed with the patient sitting on a high table. The brace is applied in the same position. After cleansing the arm a layer of stockinet is carefully rolled over the extremity, extending from just below the elbow to the level of the acromium process. The appropriate size of the brace is se- lected by measuring the length of the upper arm from approximately one inch below the axilla to just above the lateral condyle of the humerus. The brace is applied over a layer of stockinet. The brace should not press superiorly against the axilla because it will produce discom- fort and sufficient pressure to lacerate the skin and forces the proximal humerus into an abducted position. A varus angular deformity then occurs. The collar and cuff is applied holding the elbow at 90 degrees. Exercises, similar to those carried out during the cast immobilization period should continue (Figure 2.5)…………….. Figure 2.10. a) Drawing depicting the origin and insertion of the elbow flexors. At the time of the fracture the muscle fibers coil as the bony fragments rotate. Their subsequent contraction uncoils the muscle, correcting the rotary bony deformity.

Figure 2.6.

If the pa-

tient leans

on the

elbow it

can cause a

varus

angulation

to occur.

Whenever possible, the cast should be

removed with the patient sitting on a high table.

The brace is applied in the same position. After

cleansing the arm a layer of stockinet is carefully

rolled over the extremity, extending from just

below the elbow to the level of the acromium

process. The appropriate size of the brace is se-

lected by measuring the length of the upper arm

from approximately one inch below the axilla to

just above the lateral condyle of the humerus.

The brace is applied over a layer of

stockinet. The brace should not press superiorly

against the axilla because it will produce discom-

fort and sufficient pressure to lacerate the skin

and forces the proximal humerus into an abducted

position. A varus angular deformity then occurs.

The collar and cuff is applied holding the elbow

at 90 degrees. Exercises, similar to those carried

out during the cast immobilization period should

continue (Figure 2.5)……………..

insertion of the elbow flexors. At the time of the fracture the muscle fibers coil as the bony

fragments rotate. Their subsequent contraction uncoils the muscle, correcting the rotary

bony deformity.
